About
VIP stands for vasoactive intestinal peptide. It is a 28 amino acid signaling molecule the body makes in the nerves, gut, lungs, and immune tissue. As a product it is best known as a compounded nasal spray used in mold-illness and CIRS recovery protocols. It is the last step in those protocols and is started only after the other treatments are done. Some clinics and telehealth sites also sell it as a general anti-inflammatory, gut, mood, and immune spray. The synthetic form is called aviptadil. Aviptadil has been tested as an intravenous and inhaled drug for serious lung disease. It is also sold abroad in an injectable combination for erectile dysfunction. VIP is not an FDA-approved medicine in the United States. The nasal spray is a prescription compounded preparation. Non-prescription powders and sprays are gray-market products.

Frequency
Four times daily as a nasal spray
Dosing
Compounded VIP nasal spray delivers 50 mcg per spray. The dose used in the CIRS protocol is one spray four times daily for 200 mcg per day. Real-world use runs from four to eight sprays a day and reaches about 400 mcg per day at the top. Frequency is then titrated down to the lowest effective dose. The community uses only the nasal spray. No subcutaneous dose is established. The injectable numbers on vendor sites have no reliable source.
Administration
- CIRS protocols place VIP as the final step and start it only after mold exposure is removed, nasal MARCoNS is cleared, and the visual contrast sensitivity test reads normal
- The first dose is given under observation with a blood pressure check because VIP is a vasodilator
- VIP is taken continuously rather than in cycles until symptoms and lab markers settle
Half-life
About 1 to 2 minutes in humans. This is the plasma half-life measured after an intravenous infusion. VIP leaves the blood within minutes. The clinical drug aviptadil is given by continuous infusion for that reason.
Side effects
- Nasal or throat irritation can occur with the spray
- Flushing, warmth, headache, dizziness, a faster heartbeat, and lower blood pressure can follow a dose because VIP widens blood vessels
- Loose stools and watery diarrhea can appear at higher exposures
- A temporary rise in the pancreatic enzyme lipase has been recorded during the CIRS protocol
How it works
VIP acts on two receptors called VPAC1 and VPAC2. Both raise the second messenger cAMP inside cells and switch on PKA signaling. VIP relaxes smooth muscle in blood vessels and airways. It lowers blood pressure, widens the blood vessels in the lungs, and can open the airways. It also drives the gut to secrete water and salts. This is the same action that produces heavy diarrhea in the rare VIP-secreting tumor called a VIPoma. Mouse and cell studies show VIP lowering inflammatory signals such as TNF-alpha, IL-6, and IL-12 and raising IL-10. A few small human studies report similar but inconsistent immune shifts. The same receptors sit on many human tumors. Animal studies show that giving VIP can speed tumor growth and weaken anti-tumor immune defenses.
Research quality
The evidence for VIP splits by route and is uneven. Its basic human pharmacology is old but solid. The very short half-life and the blood-pressure fall came from intravenous infusion studies in the 1970s. The intranasal CIRS use rests mainly on one small open-label 2013 report from the protocol's own clinical group with no randomization or blinded control. The anti-inflammatory immune effects come mostly from mouse and cell studies, with a few small human studies that do not agree well. Synthetic VIP as aviptadil has been studied by injection and inhalation for lung disease. A large placebo-controlled trial of intravenous aviptadil for COVID-19 respiratory failure found no benefit. The inhaled pulmonary hypertension studies were small and early. Wellness claims for the nasal spray trace to marketing pages rather than trials. No controlled human trial supports routine dosing for immune, gut, or anti-aging goals.
Warnings
- Compounded or gray-market VIP is not an FDA-approved product and can vary in identity, strength, sterility, and endotoxin load
- Low blood pressure, a fast heartbeat, chest pain, or fainting after a dose needs medical advice because VIP is a vasodilator
- Heavy or watery diarrhea with weakness or cramps needs evaluation because VIP can drive fluid and potassium loss
- Active or prior cancer warrants specialist guidance because many tumors carry VIP receptors and animal studies show VIP can speed tumor growth
- Pregnancy and breastfeeding safety has not been established
- Combining VIP with blood-pressure medicines or other vasodilators can add to the drop in blood pressure
