Semax is a Russian nootropic peptide derived from a fragment of the hormone ACTH but without ACTH's hormonal effect. It has been an approved prescription medicine in Russia since 1994 for stroke recovery, cognitive problems, and some eye-nerve disorders. It is stimulating and is used for focus and mental stamina. It is not approved anywhere else. It reaches people outside Russia as a gray-market research chemical used off-label as a nootropic.

Dosing

Nootropic use is intranasal at about 300 to 600 mcg a day in the morning because it is stimulating. The registered cognitive product is dosed a little higher at about 600 to 900 mcg a day. Some inject it under the skin at lower doses even though no injectable product exists. It is short-acting so users redose within the day rather than expect all-day cover. The hospital stroke protocol uses far larger doses and is not a nootropic amount. Online guides list wider figures that trace to sellers rather than the label.

Half-life

Semax clears from the blood within minutes and breaks down into smaller fragments. No reliable plasma half-life has been published. The precise figures on vendor pages are not backed by a study. Its effect lasts hours. That comes from downstream brain changes rather than the peptide staying in the blood.

How it works

How Semax works is not settled. It is often called a melanocortin MC4 activator. The one clean receptor study instead found it blocks those receptors. The receptor behind its brain effects is unknown. Its clearer effects are downstream. It raises the growth factors BDNF and NGF in rodents and sensitizes the dopamine and serotonin systems. That mix is the leading explanation for its focus and neuroprotective effects. It breaks down into a smaller fragment that is active on its own.

Research quality

Semax has real human trials. The evidence is mostly Russian and modest. Small Russian trials in stroke recovery report better outcomes and higher BDNF. It is a registered stroke and cognition drug there and is approved for some uses in children. Those trials are small, open-label, and single-country. That falls short of Western standards. Laboratory work supports its effects on BDNF, NGF, and the dopamine and serotonin systems. Its receptor is not established. Western trials and large modern studies are absent.

Warnings

  • Semax sold outside Russia is not regulated so identity, purity, and strength vary between vendors
  • It raised serotonin turnover in rodents and amplified the dopamine release caused by a stimulant so combining it with stimulant, antidepressant, or MAO-inhibitor drugs deserves care
  • Its stimulating profile could worsen psychosis, mania, or acute psychiatric symptoms
  • Avoid it in pregnancy and breastfeeding since neither has any safety data
  • Long-term safety in healthy adults is not established and human data come from short Russian courses
