About

P21 is an experimental peptide built from a fragment of the natural brain growth factor CNTF. An adamantane cage is attached to help it cross into the brain and resist breakdown. Animal studies show it boosts the growth of new brain cells and lowers the tangled tau protein seen in Alzheimer's disease. It has never been tested in humans and is not approved anywhere. It is sold as an unregulated research chemical. Its proper code is P021. That avoids confusion with an unrelated protein also called p21.

Dosing

Community use is about 250 mcg to 2 mg a day by nasal spray or under the skin. Most people land around 500 mcg to 1 mg. About 500 mcg is the most cited effective dose. Users run it flexibly with occasional days off rather than on a fixed cycle. The animal studies instead mixed it into food. That oral dosing does not translate to a simple human amount. Some online guides list several milligrams a day. That is far above what real users report.

Half-life

No human pharmacokinetic data exists for P21. It was designed to resist breakdown and cross into the brain. One review reports a plasma half-life above three hours without naming the species. No plasma half-life has been measured in people.

Side effects

  • Mild stimulation or sharper alertness reported anecdotally
  • Injection-site reactions from subcutaneous use as with any injected peptide
  • Headache or sleep changes occasionally reported but not well documented

How it works

P21's mechanism is proposed by one research group and rests on their studies. The idea is that it blocks a signal called LIF that normally holds back the growth of new neurons. It also raises the growth factor BDNF. Higher BDNF lowers an enzyme called GSK-3beta and that lowers tau buildup. The pathway pieces are well-known brain biology. Whether P21 itself drives them is less certain. The key BDNF step failed to appear in the one semi-independent test.

Research quality

P21 has never been tested in humans. All of its evidence is from animals and cell cultures. Almost all of it comes from one research group. Rodent studies report more new neurons, denser synapses, better memory, and less tau in Alzheimer's, Down syndrome, and aging models. The results are weaker on amyloid. Only its soluble form dropped and the plaques did not. The one study run largely by an outside group failed in living animals. The compound there did not raise BDNF or improve behavior. No independent lab has reproduced the main findings. A company co-founded by the inventor is developing it. That work is still in animals and no human trial has started.

Warnings

  • P21 has no human data of any kind and its safety rests entirely on animal studies from one lab
  • P21 promotes cell growth so its cancer risk is a theoretical concern and the 18 month rodent studies reported no adverse effects without looking for tumors
  • Avoid it in pregnancy and breastfeeding since there is no human safety data
  • It lowers GSK-3beta so combining it with lithium or other GSK-3beta drugs could add up in ways no study has checked
  • Gray-market powder is unregulated so identity, purity, and dose accuracy cannot be verified
